[mkgmap-dev] Commit: r1383: Take care of the case when the number of cities/regions is more

Take care of the case when the number of cities/regions is more
than 0xffff.

Map of germany now works for me.

Also modify flag setting as per Anton's understanding of it.  Will make
no practical difference unless more than 0xffffff cities.
