Hmm, there is no > amenity=doctors in the polygons file. I will add that too. Well, for what I noticed on actual usage is that doctors, hospitals and dentist get tagged themselves. All other stuff e.g. especiall care related like chiropody or similar stuff you would not find on a general purpose map, gets thrown into healthcare. Therefore I would show amenity=hospital & ( amenity=healthcare & healthcare=hospital ) at resolution 22, include amenity=docotors at 23 and add healthcare to 23 (if not 24). The reasoning has to go that important stuff is shown early.
